GOLDEN — The Jefferson County Sheriff puts the blame on himself for the first-ever escape from the Jefferson County Jail.

Travis Sandlin broke out of the facility on October 5 and was captured 11 days later.

The wire mesh screen that Sandlin pushed out was reported for repair and a work order filed five days prior to his escape.
